Acne scars are among the most challenging skin concerns to treat because they involve structural changes to the skin that no single treatment can fully address. At Rani Beauty Clinic in Renton, WA, our acne scar elimination protocol combines multiple technologies in a strategic sequence that addresses every type of scarring for the most complete improvement possible.
Understanding scar types is essential for effective treatment because different scars respond to different technologies. Ice pick scars are narrow, deep depressions that extend into the dermis. Boxcar scars are broader depressions with defined edges. Rolling scars create an undulating, wave-like surface due to fibrous bands tethering the skin to deeper tissue. Hypertrophic scars are raised areas of excess collagen. Most patients have a combination of scar types, requiring a multi-modal treatment approach.
RF microneedling is the foundation of our acne scar protocol. The insulated needles deliver radiofrequency energy directly into the scar tissue, breaking up fibrous bands and stimulating new collagen production that remodels the scarred tissue from within. Multiple sessions produce progressive improvement as each treatment builds on the collagen laid down by previous sessions. For moderate scarring, RF microneedling alone can produce sixty to seventy percent improvement.
PicoWay laser technology addresses the pigmentation component of acne scars. Post-inflammatory hyperpigmentation, the dark marks left after acne heals, often accompanies textural scarring. PicoWay's ultra-short picosecond pulses shatter melanin deposits without significant heat, making it safe for all skin tones including melanin-rich skin that is more prone to post-inflammatory pigmentation. Correcting the color of scars makes the textural component less visible even before texture-focused treatments are complete.
Chemical peels complement the deeper treatments by accelerating surface-level renewal. VI Peel removes the outer layers of scarred skin, stimulating fresh cell production that gradually replaces damaged tissue with healthier skin. The peel also improves overall skin texture and tone, creating a more even canvas that makes individual scars less prominent.
Subcision is a technique used specifically for rolling scars. A small needle is inserted beneath the scar to release the fibrous bands that tether the skin surface to the deeper tissue, causing the depression. Once released, the skin can re-drape more evenly. Subcision is typically performed before or in conjunction with RF microneedling, as the collagen stimulation from microneedling fills the space created by releasing the tethered bands.
The protocol sequence matters significantly. We typically begin with subcision for tethered rolling scars, followed by RF microneedling for collagen remodeling, with PicoWay laser sessions interspersed for pigmentation. Chemical peels are scheduled between other treatments for ongoing surface renewal. This sequencing ensures each treatment builds optimally on the previous one.
Treatment spacing allows for adequate healing and collagen development between sessions. RF microneedling sessions are typically spaced four to six weeks apart. PicoWay sessions are spaced three to four weeks apart. A complete acne scar protocol typically involves four to six RF microneedling sessions and three to four PicoWay sessions over a period of six to twelve months.
At-home care between treatments maximizes professional results. Prescription tretinoin accelerates cell turnover and supports collagen production. Vitamin C serum provides antioxidant protection and additional collagen stimulation. Niacinamide strengthens the skin barrier and helps manage post-inflammatory pigmentation. Consistent SPF protects newly treated skin from UV damage that could cause new pigmentation.
Realistic expectations are important for acne scar treatment. While our multi-treatment protocol produces the most comprehensive improvement available without surgery, complete elimination of all scarring is rarely achievable. Most patients achieve fifty to eighty percent improvement depending on scar severity and type, with the most dramatic improvement in the first three to four treatments and continued refinement in subsequent sessions.
Patience is essential because collagen remodeling is a biological process that takes months. The full benefit of each RF microneedling session develops over three to six months as new collagen forms and matures. Patients often see their most dramatic improvement between months three and nine of their protocol, even between active treatment sessions.
At Rani Beauty Clinic, we document your progress with clinical photography at each visit, providing objective evidence of improvement that day-to-day observation may not reveal.
