Acne Scar Treatment: From Ice Pick to Rolling Scars

Acne scars represent permanent structural changes in the skin that occurred during the healing of inflammatory acne lesions. At Rani Beauty Clinic in Renton, WA, we classify acne scars by type to select the most effective treatments for each patient's specific scar pattern.

Ice pick scars are narrow, deep pits that extend into the dermis, resembling small puncture wounds. They result from severe inflammatory acne that destroyed tissue in a columnar pattern. Ice pick scars are among the most challenging to treat because of their depth and narrow diameter. RF microneedling at deeper settings helps remodel the scar tissue. TCA cross, a technique where high-concentration trichloroacetic acid is applied directly into individual scars, causes controlled destruction and rebuilding. Multiple sessions are typically needed.

Boxcar scars are broader depressions with defined vertical edges, resembling rectangular boxes pressed into the skin. They result from inflammatory acne that destroyed collagen in a wider area. Boxcar scars respond well to RF microneedling, which stimulates new collagen to fill the depression from within. Chemical peels help refine the edges. Multiple treatment sessions progressively improve depth and definition.

Rolling scars create a wave-like undulation in the skin surface. They result from fibrous bands that tether the skin surface to deeper tissue, pulling the surface down. Treatment involves breaking these fibrous bands through subcision (performed by dermatologists) and then stimulating new collagen with RF microneedling to maintain the release. The combination of band release and collagen stimulation is the most effective approach.

Hypertrophic and keloid scars represent raised, excess tissue rather than depressions. These form when the healing response produces too much collagen. Treatment may include injectable corticosteroids to flatten the excess tissue, followed by laser treatments to improve texture and color. Patients with keloid tendency require careful management, as any skin injury can trigger new keloid formation.

Post-inflammatory erythema, the persistent redness that remains after acne lesions heal, is technically not scarring but is often grouped with acne scars by patients. This redness results from dilated or damaged blood vessels in the healing area. Laser Genesis and IPL effectively reduce this vascular redness. LED therapy supports healing and reduces inflammation. This typically resolves faster than structural scars.

RF microneedling is our primary treatment for most acne scar types at Rani Beauty Clinic. The combination of controlled micro-injury and radiofrequency energy creates a powerful stimulus for collagen remodeling that is particularly effective in scar tissue. Adjustable needle depth allows treatment at the specific depth where each scar originates. A series of three to six treatments spaced four to six weeks apart produces progressive improvement.

Chemical peels complement RF microneedling by addressing the surface texture component of scarring. VI Peel applied between microneedling sessions refines skin texture and promotes even cell turnover. The combination of deep collagen remodeling from microneedling and surface renewal from peels produces the most comprehensive improvement.

Realistic expectations are important. Acne scar treatment produces significant improvement, typically fifty to seventy percent reduction in scar visibility over a treatment series. Complete elimination of deep scars is not achievable with any non-surgical approach. The goal is improvement that restores confidence rather than perfection.

Non-surgical treatments typically improve scar appearance by fifty to seventy percent. Complete removal of deep scars is not achievable without surgical intervention. However, significant improvement is consistently achievable and makes a meaningful difference in skin appearance and confidence.

Acne should be well-controlled before beginning scar treatment. Active inflammatory acne can worsen with treatments like microneedling. Once breakouts are minimal for two to three months, scar treatment can safely begin.
