While this expansion has made aesthetic treatments more accessible, it has also created an environment where the quality of care varies dramatically from one clinic to the next.

As a physician who takes medical oversight seriously, I want patients to be empowered with the knowledge to identify clinics that may not meet appropriate standards of care. Here are eight red flags that should prompt serious reconsideration before booking treatment.

Red Flag 1: No Identifiable Medical Director

In Washington State, medspas must operate under the supervision of a licensed physician. If a medspa cannot clearly identify their medical director, or if the medical director's name does not appear on their website, marketing materials, or clinic signage, this is a fundamental red flag. It may indicate that the practice is operating without proper medical oversight or that the medical director's involvement is purely nominal.

Ask directly: Who is your medical director? What is their specialty? How often are they in the clinic? If the answers are vague or evasive, proceed with caution.

Red Flag 2: Providers Who Cannot Explain the Treatment

Your provider should be able to clearly explain what treatment they are recommending, how it works, what results you can expect, what the risks are, and why it is the best option for your specific concerns. If your provider cannot articulate these points or relies heavily on marketing language without substantive explanation, their understanding of the treatment may be insufficient.

Quality clinics let results and reputation drive their business. They provide thorough consultations, present options transparently, and give patients time to make informed decisions without pressure. If you feel rushed, pressured, or manipulated, that is not a clinic that has your best interests at heart.

Red Flag 4: Unrealistic Promises

Any provider who guarantees specific results is making a promise they cannot keep. Aesthetic medicine involves biological processes that vary from person to person. A responsible provider sets realistic expectations based on clinical evidence and their experience, acknowledging that individual results vary.

Be wary of claims like guaranteed results, age reversal, permanent solutions for conditions that are inherently temporary, or results that seem too dramatic to be plausible. Honest medicine involves honest expectations.

Red Flag 5: Suspiciously Low Pricing

Aesthetic treatments have real costs. FDA-cleared products like Botox have manufacturer wholesale prices that set a floor on what clinics can charge responsibly. Medical devices require significant capital investment and ongoing maintenance. Qualified providers command competitive salaries.

When a clinic prices significantly below market, the money is being saved somewhere. It may be on product quality, using diluted or counterfeit products. It may be on provider qualifications, hiring less experienced or less trained staff. It may be on medical oversight, minimizing physician involvement. It may be on sanitation and safety protocols. None of these cost savings benefit the patient.

Red Flag 6: No Complication Management Protocol

Complications, while uncommon with properly performed treatments, can occur. Burns, infections, vascular occlusion from filler, asymmetry, allergic reactions, and other adverse events are known risks of aesthetic procedures. A clinic that dismisses the possibility of complications or has no clear protocol for managing them is unprepared for the realities of medical aesthetics.

Ask what happens if something goes wrong. Is a physician available? Is there an emergency protocol? Can you reach someone after hours? A clinic that takes safety seriously will have clear answers to these questions and will appreciate that you asked.

Red Flag 7: Outdated or Unverified Technology

Ask what devices the clinic uses and verify that they are FDA-cleared for their stated indications. Some clinics use devices that are not FDA-cleared, imported from overseas, or so outdated that their safety and efficacy no longer meet current standards. Others use genuine devices but perform treatments outside the device's cleared indications without informing patients.

You have every right to know who is treating you, what they are using, what it costs, and what could go wrong. Any clinic that makes you feel uncomfortable for asking these questions is not a clinic that deserves your trust.

The opposite of these red flags is straightforward. A quality medspa has an actively involved, board-certified medical director. Providers who can articulate the science behind every treatment they perform. No high-pressure sales tactics. Honest, realistic expectations. Fair pricing that reflects the genuine cost of quality care. Clear complication management protocols. FDA-cleared, well-maintained technology. And a culture of transparency that welcomes every question.
