Before you arrive, there is minimal preparation required. We ask patients to come with clean skin, free of makeup, sunscreen, or heavy moisturizers on the treatment area. You do not need to avoid any medications or supplements beforehand. Some patients take an over-the-counter pain reliever like ibuprofen an hour before their appointment as a precaution, though most find it unnecessary.

When you arrive, we will take you to a treatment room and cleanse the skin thoroughly. Your provider will review the treatment plan that was developed during your consultation, confirm the areas being treated, and answer any last-minute questions. A thin layer of ultrasound gel is applied to the treatment area, similar to what you would experience during a diagnostic ultrasound. This gel helps the Sofwave handpiece maintain consistent contact with the skin and ensures efficient energy delivery.

The treatment itself is where the experience gets interesting. Your provider will move the Sofwave handpiece across the treatment area in a systematic grid pattern. Each placement of the handpiece delivers a brief pulse of ultrasound energy. You will feel a sensation of heat building beneath the skin, followed by a brief moment of more intense warmth as the energy reaches therapeutic temperature. This lasts approximately one to two seconds per pulse.

The sensation is often described as a brief, deep warmth or a hot prickling feeling beneath the surface. It is not the sharp, shooting pain that some patients experience with Ultherapy. Most patients describe it as tolerable, though there are moments of discomfort, particularly in areas with less subcutaneous fat like the forehead and along the jawbone. The cheeks and under-chin area tend to be more comfortable.

The Sofcool system is running continuously during treatment, blowing cooled air onto the skin surface. This serves two purposes: it protects the epidermis from thermal damage and it makes the treatment significantly more comfortable than it would be without cooling. You will feel the cool air between pulses, which provides relief and helps manage any warmth that builds up.

A full-face treatment typically takes 30 to 45 minutes. A targeted area like the brow or jawline alone takes 15 to 20 minutes. Throughout the treatment, your provider will check in with you about your comfort level and can adjust the treatment pace if needed.

Immediately after treatment, the gel is removed and your skin is cleansed. Most patients notice mild pinkness or slight warmth in the treated area, similar to a mild sunburn sensation. This typically resolves within 30 minutes to two hours. Some patients notice mild swelling along the jawline that lasts a day or two. There is no bruising, no peeling, and no open wounds.

You can apply makeup immediately after treatment. You can exercise, shower, and resume all normal activities right away. The only aftercare recommendation is to apply sunscreen diligently in the days following treatment, which you should be doing regardless.

In the days following treatment, you may notice a subtle firmness or tightness in the treated area. This is the initial collagen contraction effect and it is a good sign. Over the following weeks and months, the real results develop as new collagen is produced. Most patients start noticing visible improvement around the four to six week mark.
