If you are researching non-surgical skin tightening, you have probably come across both Sofwave and Ultherapy. Both treatments use ultrasound energy to stimulate collagen production and tighten skin. Both are FDA-cleared. Both require no downtime. So what is the actual difference, and which one should you choose?
The fundamental difference is in the technology. Ultherapy was the first FDA-cleared ultrasound device for non-invasive skin lifting, introduced in 2009. It uses micro-focused ultrasound with visualization, known as MFU-V, which delivers concentrated energy to specific focal points at depths of 1.5mm, 3.0mm, and 4.5mm. The visualization component allows the provider to see the tissue layers on an ultrasound screen and target energy to precise depths. Ultherapy's deepest setting reaches the SMAS layer, the same tissue layer that surgeons manipulate during a facelift.
Sofwave, cleared by the FDA in 2021, uses a fundamentally different approach called SUPERB technology, Synchronous Ultrasound Parallel Beam Technology. Instead of focusing energy at a single point, Sofwave delivers seven parallel beams of ultrasound energy simultaneously to a uniform depth of 1.5mm in the mid-dermis. This creates a broader, more even distribution of thermal energy across the treatment area.
The comfort difference is significant. Ultherapy has a well-documented reputation for being uncomfortable. Many patients describe the treatment as painful, particularly in areas with thinner skin or more nerve endings like the forehead and jawline. Some providers administer nerve blocks, oral pain medication, or even sedation to help patients tolerate the procedure. Sofwave, by contrast, is generally described as much more tolerable. Ultherapy proponents argue that its ability to reach the deeper SMAS layer (4.5mm) produces a more significant lifting effect. Sofwave proponents argue that the mid-dermal targeting is sufficient for most patients and that the more even energy distribution produces more consistent results with fewer complications. Clinical studies for both devices show meaningful improvement in skin laxity, though direct head-to-head comparison studies are limited.

Treatment time also differs. A full-face Ultherapy session typically takes 60 to 90 minutes. A full-face Sofwave session takes 30 to 45 minutes. The shorter treatment time, combined with the more comfortable experience, makes Sofwave a more practical option for patients with busy schedules.
