A patient who is not seeing expected results on semaglutide after adequate titration may respond better to tirzepatide, which acts on both GLP-1 and GIP receptors (dual-agonist mechanism). The reverse can also be true.

<h2>Semaglutide vs. Tirzepatide: Key Differences</h2>

Semaglutide is a GLP-1 receptor agonist. It works on a single receptor pathway to reduce appetite, slow gastric emptying, and improve blood sugar regulation. It has extensive clinical data behind it and is well-established in weight management.

Tirzepatide is a dual GLP-1 and GIP receptor agonist. It activates two receptor pathways, which some research suggests may produce greater weight loss in certain patient populations. Clinical trials have shown tirzepatide producing slightly higher average weight loss percentages compared to semaglutide, though individual responses vary significantly.

Switching GLP-1 medications is not as simple as stopping one and starting the other at the same dose. Your provider will determine the appropriate starting dose of the new medication based on your current dose, how long you have been on therapy, and your individual response.

In most cases, there is a brief transition period where the new medication is titrated up gradually, similar to when you first started GLP-1 therapy. This titration helps minimize GI side effects and allows your body to adjust to the new medication.

During the transition, you may experience a temporary return of mild side effects like nausea or decreased appetite as your body adjusts. Weight loss may fluctuate during the first few weeks as the new medication takes full effect. These are normal aspects of the transition and typically resolve within two to four weeks.
