<h2>What Your Thyroid Does</h2>
The thyroid is a butterfly-shaped gland at the base of your neck that produces hormones regulating your metabolism, the rate at which your body converts food into energy. Thyroid hormones affect virtually every system in your body, including heart rate and cardiovascular function, body temperature regulation, energy production, mood and cognitive function, hair, skin, and nail health, digestive function, and of course, weight regulation.
When thyroid function is optimal, these systems run smoothly. When it is not, the effects can be widespread and significantly affect quality of life.
<h2>Key Thyroid Panel Markers</h2>
<strong>TSH (Thyroid-Stimulating Hormone):</strong> TSH is produced by the pituitary gland and signals the thyroid to produce hormones. High TSH indicates the thyroid is underperforming (hypothyroidism), so the pituitary is working harder to stimulate it. Low TSH indicates the thyroid is overproducing (hyperthyroidism), so the pituitary has backed off. Normal TSH is typically 0.4 to 4.0 mIU/L, though optimal ranges are debated among clinicians.
<strong>Free T4 (Thyroxine):</strong> The primary hormone produced by the thyroid. Free T4 measures the unbound, active form available for use by the body. Low Free T4 with high TSH confirms hypothyroidism.
<strong>Free T3 (Triiodothyronine):</strong> The most active thyroid hormone, converted from T4 in peripheral tissues. Free T3 is critical for metabolic function, but is not always included in basic thyroid screening. Low T3 can cause symptoms even when T4 and TSH appear normal.
<strong>Thyroid Antibodies (TPO and TG antibodies):</strong> These markers identify autoimmune thyroid disease, the most common cause of hypothyroidism (Hashimoto's thyroiditis). Elevated antibodies indicate that the immune system is attacking the thyroid gland.
<strong>Reverse T3:</strong> This inactive form of T3 can accumulate during periods of stress, illness, or caloric restriction. High reverse T3 relative to free T3 can cause hypothyroid symptoms even when standard markers look normal.
<h2>Hypothyroidism and Weight</h2>
Hypothyroidism (underactive thyroid) slows metabolism, making weight gain easier and weight loss more difficult. Symptoms include fatigue, cold intolerance, weight gain or difficulty losing weight, dry skin and hair, constipation, depression, brain fog, and hair loss.

<h2>The "Normal" Range Debate</h2>
Many clinicians and patients are frustrated by the broad reference range for TSH. A TSH of 0.5 and a TSH of 3.5 are both "normal" by standard ranges, but they represent very different levels of thyroid function. Some patients feel their best with TSH levels in the lower part of the reference range (1.0 to 2.0), while others are comfortable at higher levels.

<h2>Thyroid and GLP-1 Therapy</h2>
GLP-1 medications carry a boxed warning about thyroid C-cell tumors based on animal studies. While this has not been demonstrated in humans, patients with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 should not take GLP-1 medications. Thyroid screening before starting GLP-1 therapy helps identify any pre-existing thyroid conditions.
