Phase 1: Evaluation (Weeks 1-2). Your journey begins with a thorough consultation with Dr. Landfield. We review your complete health history, previous weight loss attempts, current medications, and goals. In-house blood work establishes your metabolic baseline. Based on this evaluation, your personalized treatment plan is developed including medication selection, starting dose, and nutritional guidance.
Phase 2: Initiation (Weeks 2-4). Treatment begins at the lowest dose of your selected GLP-1 medication. During this phase, the primary goal is establishing tolerability and allowing your body to adjust. You may notice modest appetite changes. Side effects, primarily mild nausea, are most common during initiation and typically decrease over time. Follow-up at the end of this phase assesses your response and determines the next dose adjustment.
Phase 3: Titration (Months 1-3). Your dose is gradually increased according to the titration schedule. As the dose increases, appetite suppression becomes more noticeable and weight loss begins in earnest. Most patients lose 3 to 5 percent of body weight during this phase. Nutritional habits are being established and exercise is being integrated. Regular follow-ups monitor your progress and manage any side effects.
Phase 4: Active Weight Loss (Months 3-12). This is the phase of most significant weight loss. At therapeutic doses, patients typically lose 1 to 2 percent of body weight per month. Lab work is repeated at the 3 and 6 month marks to track metabolic improvements. Nutritional strategies are refined based on your experience. Exercise intensity and consistency are building. This phase produces the results that motivate continued commitment.
Phase 5: Optimization (Months 12-18). Weight loss typically slows as you approach a new metabolic set point. This is normal and expected. The focus shifts from rapid weight loss to body composition optimization. Strength training becomes increasingly important for muscle preservation and metabolic health. Lab work may show significant improvements in glucose, lipids, and other metabolic markers.
Phase 6: Maintenance (Ongoing). The transition to maintenance is individualized. Some patients continue medication at a lower dose. Others transition off medication with close monitoring. Regardless of medication status, maintenance requires continued healthy eating habits, regular exercise, periodic check-ins with your provider, and ongoing attention to the lifestyle factors that support your new weight.
Nutrition throughout the journey: Protein intake is prioritized at every phase (0.7 to 1.0 grams per pound of ideal body weight). The plate method (half vegetables, quarter protein, quarter complex carbs) guides meal composition. Hydration targets are maintained throughout. Nutritional guidance is adjusted at each phase based on your response and needs.
Exercise throughout the journey: Phase 1-2: gentle movement, walking. Phase 3-4: building toward 150 minutes of moderate activity per week with introduction of strength training. Phase 5-6: established routine with emphasis on strength training for muscle preservation and metabolic health.
Wellness injections as support: B12 and vitamin D3 injections support energy and metabolic function throughout the weight loss process. These supplemental treatments address nutritional needs that may be increased during weight loss.
