First-Timer Guide
First Time Dermal Fillers: What to Expect
Expert guidance from Dr. Alexander Landfield, Board-Certified Neurologist
Dermal fillers restore volume, smooth lines, and enhance facial contours with results you can see immediately. If you're considering fillers for the first time, it's natural to have questions — and we want you to feel informed and confident. Every filler treatment at Rani Beauty Clinic is performed under the medical direction of Dr. Alexander Landfield, board-certified neurologist.
Pain Level
Most patients rate discomfort at 3–4 out of 10 — topical numbing plus lidocaine in the filler makes the process very manageable
Duration
30–60 minutes depending on the number of areas treated
Downtime
Minimal — mild swelling for 2–5 days, possible bruising for 5–10 days (easily concealed with makeup after 24 hours)
Sessions
Most areas require one session, with results lasting 6–18 months depending on the filler type and area treated
Before Your Appointment
- 1Avoid blood-thinning medications and supplements (aspirin, ibuprofen, fish oil, vitamin E) for 48 hours before treatment to minimize bruising
- 2Avoid alcohol for 24 hours prior to your appointment
- 3Come with a clean face — minimal or no makeup on the treatment areas
- 4Eat a light meal beforehand so you're comfortable during the procedure
- 5If you have a history of cold sores and are treating the lip area, notify your provider — antiviral medication may be recommended
During Your Treatment
Facial Assessment
Your provider performs a thorough facial analysis, discussing areas of volume loss, asymmetry, and your aesthetic goals. Photos may be taken for treatment planning and comparison.
Treatment Planning
Your provider recommends the optimal filler type and volume for your goals. Different areas may benefit from different filler consistencies — your plan is fully customized to your facial anatomy.
Numbing & Preparation
A topical numbing cream is applied to the treatment areas. Most modern fillers also contain lidocaine (a built-in anesthetic) for added comfort during injection.
Injection
Using precision technique, your provider injects filler into the targeted areas. You may feel mild pressure or a slight pinch. Your provider continuously assesses symmetry and shape throughout the process.
Sculpting & Assessment
Gentle massage may be used to shape the filler and ensure smooth, even distribution. Your provider will show you the results and confirm you're happy before you leave.
Aftercare Review
You'll receive detailed aftercare instructions including what to expect in the days ahead and when to schedule your follow-up.
After Your Treatment
- Mild swelling and possible bruising are normal and typically peak at 24–48 hours before resolving over 5–7 days
- Apply cold compresses (not direct ice) to the treated area for 10 minutes on, 10 minutes off during the first few hours
- Avoid touching, pressing, or massaging the treated areas for 48 hours unless instructed by your provider
- Sleep face-up and slightly elevated for the first 1–2 nights to minimize swelling
- Avoid strenuous exercise, alcohol, and excessive heat (saunas, hot yoga) for 24–48 hours
When Will I See Results?
Immediate volume restoration visible right away, with final results settling at 2 weeks once swelling fully resolves
Frequently Asked Questions
Do dermal fillers hurt?
With topical numbing cream and the lidocaine built into modern fillers, most patients find the treatment very comfortable. You'll feel some pressure and a mild pinch during injection, but significant pain is rare. The lip area tends to be more sensitive than cheeks or jawline. Most patients rate their discomfort at 3–4 out of 10.
How long do dermal fillers last?
Filler longevity depends on the product used, the area treated, and your individual metabolism. Lip fillers typically last 6–9 months, cheek fillers 12–18 months, and jawline fillers up to 18 months. Hyaluronic acid fillers are fully reversible — if you ever want to adjust your results, the filler can be dissolved with an enzyme injection.
Will I look overdone or unnatural after fillers?
At Rani Beauty Clinic, our philosophy is 'enhance, never exaggerate.' We focus on restoring natural volume and creating harmonious proportions rather than dramatically altering your features. Your provider will recommend conservative amounts — you can always add more at a follow-up appointment, but you can't take away over-injection.
How much swelling should I expect after fillers?
Swelling is a normal part of the healing process and varies by treatment area. Lip fillers tend to produce the most swelling, often making lips appear larger than the final result for 2–5 days. Cheek and jawline fillers typically have less dramatic swelling. The key takeaway: don't judge your final results until 2 weeks post-treatment.
Can dermal fillers be reversed if I don't like them?
Yes — hyaluronic acid fillers (the type most commonly used) can be dissolved at any time with an enzyme called hyaluronidase. The reversal process takes just a few minutes, and the filler dissolves within 24–48 hours. This reversibility is one of the reasons we recommend hyaluronic acid fillers for first-time patients.
