The GLP-1 hormone in nature: Glucagon-like peptide-1 is a hormone produced by L-cells in the small intestine in response to food intake. It is part of the incretin system, a hormonal feedback loop that connects your gut to your brain and pancreas. Natural GLP-1 has a very short half-life of approximately two minutes, as it is rapidly degraded by the enzyme DPP-4.

GLP-1 receptors and their locations: GLP-1 receptors are found in multiple organs, which explains the wide-ranging effects of GLP-1 medications. In the pancreas, activation of GLP-1 receptors on beta cells stimulates insulin secretion in a glucose-dependent manner (meaning insulin is only released when blood sugar is elevated). In the stomach, GLP-1 receptor activation slows gastric emptying, prolonging the feeling of fullness after eating.

How semaglutide works: Semaglutide is a modified GLP-1 molecule that has been engineered to resist DPP-4 degradation and to bind to albumin in the blood, dramatically extending its half-life to approximately seven days. This allows once-weekly dosing. Semaglutide binds to and activates GLP-1 receptors throughout the body with the same biological effects as natural GLP-1 but with sustained, continuous activity rather than the brief pulses that occur naturally after meals.

How tirzepatide works: Tirzepatide is a dual-agonist that activates both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors. GIP is another incretin hormone that complements GLP-1 by improving insulin sensitivity, potentially enhancing fat metabolism, and providing additional central appetite regulation. The dual mechanism may explain why clinical trials show greater weight loss with tirzepatide compared to semaglutide for some patients.

The brain appetite center: The most important effect for weight management is the action on the hypothalamus and brainstem appetite centers. GLP-1 receptor activation in these areas reduces hunger signals, increases satiety signals, and decreases the reward value of highly palatable foods. Patients describe this as the constant background noise of hunger being turned down. Food becomes less psychologically compelling, making it easier to eat appropriate portions and resist cravings.

Metabolic effects beyond appetite: GLP-1 medications improve insulin sensitivity, which helps the body process blood sugar more efficiently. They reduce hepatic (liver) glucose production. They improve lipid metabolism. These metabolic improvements contribute to better overall health beyond the weight loss itself and are why GLP-1 medications show cardiovascular benefit in clinical trials.

The titration rationale: GLP-1 medications are started at low doses and gradually increased over weeks to months. This titration approach allows the body to adapt to the medication and minimizes gastrointestinal side effects, which occur because GLP-1 receptors in the gut slow gastric emptying. Starting at full dose would produce intolerable nausea in most patients. The gradual increase allows the gut receptors to adapt while the appetite-suppressing brain effects build progressively.
